Social Work
Working as part of Salem Manor’s Interdisciplinary team, the Social Worker supports the Resident’s and family with the psychosocial adjustment to Long Term Care. Their role in the provision of resident care includes: building relationships, assessment, crisis intervention, education, advocacy, resources finding, and consultation. The social Worker is also responsible for connecting with families regarding admissions, tours and Care Conferences.
